Tralasciando la spettacolarità della canzone, mi serve un aiuto su una cosa. La storia e' abbastanza semplice, ma non riesco a capire bene questo passaggio:

But I know that it's on my mind
That my doctor said I've done it before.

Sta cercando qualcuno che lo aiuti giusto? Ma perchè viene fuori con questo:

So che è nella mia mente, (che) il dottore ha detto che l'ho già fatto(o l'ho fatto prima).

Cosa ha già fatto prima? Subito ho pensato che inconsciamente è stato lui ad uccidere, senza rendersene conto, un po' come in Killers, ed è questo ciò che gli sta dicendo il dottore...ma non sta in piedi come ipotesi.
Che ne dite?
